Job Summary and Qualifications

This position supports the mission of Swedish Medical Center of optimizing the quality of life of all those served by expanding beyond the traditional focus on medical care to meet the needs of the whole person. The Clinical Coord Case Manager is responsible for delivery of care coordination and delivery of quality patient care with cost effective utilization of resources. This position supports collaboration and communication among the patient/family, physician and the entire health care team regarding the patient’s plan of care and continued needs post-discharge. The Clinical Coord Case Manager will communicate daily with the discharge planners, social work staff, and UR RN’s to determine the plan of care and communicate this to the physicians. Communication with other members of the interdisciplinary team will be ongoing to determine the post-acute care needs of the patient. The communication with post-acute vendors; LTAC, sub-Acute Rehab, SNF, HHC etc. will be critical to ensure appropriate and timely referrals are initiated. The Clinical Coord Case Manager will be the primary contact for the discharge planners and the social workers to discuss issues that arise with patients, families, and physicians. Avoidable days will be recorded as a team effort with UR nursing, DCP and SW staff. The Clinical Coord Case Manager’s will be required to report on length of stay issues and barriers to discharge with administration on a weekly or as needed basis.

Summary

This position is eligible for the Education Debt Reduction Program (EDRP) – a student loan payment reimbursement program. Qualifications

To qualify for this position – you must meet the basic requirements as well as any additional requirements (if applicable) listed in the job announcement. Applicants pending the completion of training or license requirements may be referred and tentatively selected but may not be hired until all requirements are met. Currently employed physician(s) in VA who met the requirements for appointment under the previous qualification standard at the time of their initial appointment are deemed to have met the basic requirements of the occupation.

Basic Requirements:

  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy
  • Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine
  • The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed
  • Current – full and unrestricted license to practice medicine or surgery in a State – Territory – or Commonwealth of the United States – or in the District of Columbia
  • Residency Training: Physicians must have completed residency training – approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification
  • (NOTE: VA physicians involved in academic training programs may be required to be board certified for faculty status.) Approved residencies are: (1) Those approved by the Accreditation Council for Graduate Medical Education (ACGME) – b) OR [(2) Those approved by the American Osteopathic Association (AOA) -OR (3) Other residencies (non-US residency training programs followed by a minimum of five years of verified practice in the United States) – which the local Medical Staff Executive Committee deems to have provided the applicant with appropriate professional training and believes has exposed the physician to an appropriate range of patient care experiences
  • Residents currently enrolled in ACGME/AOA accredited residency training programs and who would otherwise meet the basic requirements for appointment are eligible to be appointed as “Physician Resident Providers” (PRPs)
  • PRPs must be fully licensed physicians (i.e. – not a training license) and may only be appointed on an intermittent or fee-basis
  • PRPs are not considered independent practitioners and will not be privileged
  • rather – they are to have a “scope of practice” that allows them to perform certain restricted duties under supervision
  • Additionally – surgery residents in gap years may also be appointed as PRPs
  • Proficiency in spoken and written English
  • Additional Requirement: Fellowship training in Addiction Psychiatry or Addiction Medicine required
  • Experience in treating Opioid Use Disorders required
  • Preferred Experience: ABPN Addiction Board Certification

  • Psychiatrists certified in Addiction Psychiatry or Addiction Medicine are expected to provide health care services to veteran patients enrolled at the Fayetteville NC VA Coastal Healthcare System
  • Services will include but are not limited to performing initial psychiatric evaluations – conducting medication management clinics and providing appropriate treatment in collaboration with a multidisciplinary Pain Management Clinic Team in the management of Veteran patients with complex/chronic pain and opioid use disorder on a predominantly outpatient basis
  • Conditions requiring ancillary services such as psychological evaluation – other substance abuse treatment – psychotherapy for victims of sexual and/or combat-related trauma – and other mental health conditions anticipated to respond to non-pharmacological interventions will be referred to those services within the Behavioral Health Service Line (BHSL) when appropriate
  • Conditions requiring specialty care will be referred to specialty clinics within the VA health care system
  • Physicians will not refer the patient to outside providers unless reviewed for eligibility by Health Administration Staff and appropriately authorized
  • Treatment and professional duties as outlined in the incumbent’s clinical privileges – as noted in our credentialing process
  • some of which are listed below: Initial Psychiatric Evaluation of veterans to establish diagnosis and management plan – to develop a treatment plan including medication management – ordering of pertinent laboratory and radiology tests – and disposition of these patients
  • Obtain appropriate consultation from specialists/ancillary services and communicate pertinent medical information to other health care providers/institutions
  • Enter all diagnostic and treatment information – and related reports into the computerized patient record system (CPRS) on the day of the visit
  • Documentation of health care provided – including all physician orders – must satisfy Department of Veteran Affairs and local VAMC policies/procedures – as well as meet external accreditation agency requirements for safe delivery of patient care
  • The physician must possess computer skills to access the CPRS as utilized by the VAMC
  • Orientation to CPRS will be provided by the VAMC
  • Prescribe medications for patients according to the established drug formulary at this facility
  • Variance from the formulary should occur only when clinically indicated and in compliance with the procedures established
  • Serve as treatment and consulting Psychiatrist/prescriber on the Pain Management Clinic Team
  • Any administrative requirement for referral and/or transfer to another medical facility will be completed by Health Administration Service personnel – and only as approved by the Chief of Staff or designee prior to transfer to an non-VA facility
  • Perform services within the VA policies and procedures and the regulations of the Medical Staff By-Laws of the VAMC
  • To provide psychiatric consultation to other services at the VAMC and associated community clinics when required

Summary:

Bon Secours Medical Group is seeking a psychiatrist who will be responsible for direct patient care in behavioral health and chemical dependency units at Bon Secours Community Hospital in accordance with CMS, Joint Commission, OASAS, and OMH standards and guidelines. Candidate will also provide consultation liaison to medical floors, nursing homes, and emergency department, and perform telepsychiatry services with St. Anthony Community Hospital.

RESPONSIBILITIES:

* Provides primary patient care on inpatient mental health unit and chemical dependency unit

* Attends court proceedings as needed for legal proceedings associated with patient care

* Serves as consultant and liaison with medical floors, emergency department, and nursing homes

* Provides tele-psychiatric services with St. Anthony Community Hospital

* Provides on call services one night a week and one weekend a month

* Completes peer reviews with insurances carriers to determine medical necessity and prior authorizations for medications/care

* Oversees compliance with electronic records and accurate billing for each encounter

* Works with administrator regarding policy and procedure, utilization, and productivity

* Meets Psychiatric Core Measures accurately and timely

* Completes psychiatric evaluations and discharge summaries within 24 hours

* Perform other duties as assigned

QUALIFICATIONS / REQUIREMENTS:

Experience: Completion of psychiatric residency; inpatient treatment preferred

Education: M.D. with board certification in psychiatry and/or addiction preferred
